Abstract
The present article highlights the Italian reality about safe administration of look-alike/sound-alike drugs in hospital wards. The 40% of medication errors is ascribable to exchange of drugs. The international literature shows that the problem is underestimated. The Italian Ministry of Health has issued specific recommendations on “LASA drugs and patient safety”, this project is addressed to hospitals
